Remember that icd-9 coding has not changed. You should still choose
icd-9 codes according to official guidelines.
Repeatedly in the final HHPPS ruling, CMS states that coding should be done
according to coding guidelines. And,
"It is significant to note that the logic for determining both the primary and
secondary diagnoses remains unchanged
(see the OASIS Implementation Manual, Definition Section of M0230/240 as well
as Attachment D to Chapter 8).
The primary diagnosis is determined based on the condition most related to the
current plan of care. This diagnosis
may or may not be related to a patient’s recent hospital stay but must relate
to the services rendered by the HHA."

What has changed? Basically, some codes that carried case-mix
weight, no longer have weight. Other codes which had
no case-mix weight do now. The weight values have changed and the
way icd-9 case-mix weights combine with other
scores has changed. How you choose icd-9 codes has not
changed. You need a coding manual and an understanding
of the coding guidelines. And you can get everything you need on
this page.
